ประชาสัมพันธ์ การยืมและคืนชุดครุยวิทยฐานะของอาจารย์ และ เจ้าหน้าที่มหาวิทยาลัยพายัพ ในพิธีประสาทปริญญาบัตร ครั้งที่ 46 Application for borrowing graduation gown and robe for PYU faculty members 46th Commencement ceremony: Academic year 2022

     ตามที่มหาวิทยาลัยพายัพ กำหนดจัดพิธีประสาทปริญญาบัตร ครั้งที่ 46 ประจำปีการศึกษา 2565 ในวันเสาร์ที่ 18 พฤศจิกายน 2566 นั้น คณะอนุกรรมการฝ่ายครุยวิทยฐานะคณาจารย์ และ เจ้าหน้าที่ มหาวิทยาลัยพายัพ ขอประชาสัมพันธ์ให้บุคลากรที่ประสงค์จะใช้บริการชุดครุยจากห้างหุ้นส่วนจำกัด เดอะ เกร็ท (สาขาเชียงใหม่)โดยทางร้านฯ ยินดีให้บริการยืมชุดฯ ฟรี โดยดาวน์โหลดแบบฟอร์ม ได้ที่นี่

     คณะอนุกรรมการฯ จึงขอแจ้งกำหนดการรายละเอียดการยืนยันข้อมูล การรับ - ส่งคืนชุดครุย ให้แก่บุคลากร โดยได้แนบเอกสารแบบขอใช้บริการยืมชุดครุยฯ ซึ่งขอความกรุณา กรอกข้อมูลเพื่อยืนยันการยืมชุดครุย และ ส่งแบบยืนยันทั้งฉบับกลับมายัง บัณฑิตวิทยาลัย ภายในวันที่ 17 ตุลาคม 2566 รายละเอียดดังตาราง


หมายเหตุ
1. หากส่งคืนหลังวันที่ 30 พฤศจิกายน 2566 ทางร้านคิดค่าปรับเป็นเงินจำนวน 3,000.- บาท (สามพันบาทถ้วน)
โดยท่านยินยอมให้มหาวิทยาลัยหักจากเงินเดือนของท่านและลงนามตามแบบฟอร์มที่แนบมาพร้อมนี้
2. กรณีทำหมวกและพู่หาย มีค่าใช้จ่ายดังนี้ ค่าหมวก ใบละ 100.- บาท/ค่าพู่ อันละ 50.- บาท
3. ขอสงวนสิทธิ์สำหรับคณาจารย์หรือเจ้าหน้าที่ ที่ยังไม่ได้คืนชุดครุยให้กับทางร้านเมื่อปีการศึกษาที่ผ่านมา

     According to the announcement of PYU on the 46th of the Academic year 2565 Commencement ceremony which will be on Saturday November 18th, 2023, the graduation gown service committee would like to inform all PYU staff who want to borrow the graduation gown that THE GREAT SHOP (Chiang Mai Branch) offers a free lending service.

     The graduation gown service committee would like to inform you about the details of the borrowing and returning of gowns for personnel. In the attachment, please find a request form for borrowing the gown, kindly fill in and send back to the Graduate School (GS) at PC207, Pentecost Building (Mae Khao campus) within October 17th, 2023. For more detail, please see in the attached schedule below.



Remark :
1. Late return fee is 3,000 baht (after November 30th , 2023)
2. Lost fee : cap = 100 baht of each , tassel = 50 baht of each
3. Faculty Staffs who previously have not returned the apparel (last years) are not eligible to this service.
